Community Living
Supported Living services are provided in a variety of community settings for up to 24 hours per day.
Independent Living services are provided for those individuals who need less than 24 hour support.
Family Living services are furnished by a companion, surrogate or natural family for up to 24 hours per day.
These services are intended to provide the support needed to live a
satisfying life in the community and may include assistance with money
management, meal planning and preparation, health monitoring and
maintenance, personal care, household care, planning and participation
in recreation and leisure activities, medication administration and/or
assistance and nursing support, and developing effective self-advocacy
skills.
Individuals may receive these services in their family home or in their
own home. In all environments, these services focus on increasing,
maintaining, or promoting independent functioning, social and
relationship skill development, and full participation in the community.
A 24-hour on-call system provides emergency backup support for all individuals receiving Supported, Independent and Family Living services in order to assure the health safety of the individual.
These services are designed around each individual's unique needs and
desires and promote the individual's capacity to live independently and
as a full member of the community.
Who is Eligible?
To receive services, the individual must:
- Have mental retardation or a specified related condition and a developmental disability that began before age twenty-two
- Need a combination of special, interdisciplinary services that are life long or of an extended duration and are individually planned
- Meet New Mexico Medicaid financial and medical criteria for care and be allocated for services by the Developmental Disabilities Supports Division of the Department of Health
- Be a resident of New Mexico
