Day Habilitation Services/Community Membership
Staff support individuals in a daily schedule of meaningful and individually oriented activities. An individual participates in a variety of community life experiences that may include leisure/recreational activities, self advocacy activities, cultural activities, adult educational activities, personal care skill building, employment exploration and skill development, volunteering, and gaining a greater familiarity with the community and how to access resources.
Through participation in Adult Habilitation/Community Membership, individuals choose activities of interest to him/her, with emphasis on achieving specific goals and developing relationships that help them move towards greater independence.
In addition to the wide array of daily activities, other supports such as medication assistance, nursing oversight, and nutritional counseling are offered.
These services are designed around the individual's unique needs and desires and promote maximum participation in community life.
Who is Eligible?
To receive services, the individual must:
- Have mental retardation or a specified related condition and a developmental disability that began before age twenty-two
- Need a combination of special, interdisciplinary services that are life long or of an extended duration and are individually planned
- Meet New Mexico Medicaid financial and medical criteria for care and be allocated for services by the Long Term Services Division of the Department of Health
- Be a resident of New Mexico
