Customized Community Supports are intended to provide individualized activities that support each person’s definition of a meaningful day and are intended to promote inclusion by helping community members discover the unique gift and talents individuals possess. The services are designed to promote independence and relationships,
Although Service Coordination is not a stand alone service it is an essential component of the provision of quality services. Because through Freedom of Choice individuals may have a variety of providers offering a wide range of services, it is extremely important to have a team member focused on bringing all that together. Individuals begin their journey when intake is completed by the Service Coordinator with the individual and his/her family or guardian. During the intake interview, important information is gathered about the person that is instrumental in the development of the Individual’s Service Plan and the acquisition of needed supports. In collaboration with the Case Manager, the Service Coordinator schedules formal team meetings, communicates and works closely with families, guardians and team members. Service Coordinators function to assure needed services are provided whenever possible, community resources are utilized and the plan developed with the individual is implemented, reviewed, and modified. The Service Coordinator gathers essential information, evaluates the individual’s progress and provides reports that help guide decision making. They are the internal team leader.
Behavior Support Consultation
Occasionally, individuals may exhibit some behaviors that can interfere with their fully participating in an inclusive life in their communities and in establishing a full range of relationships. Behavior Support Consultants are highly trained professionals who can guide the individual and their team in understanding the factors that are influencing behavior, develop support strategies to lessen the negative impact and promote autonomy, and enhance staff competency in preventing these negative behaviors or safely intervene when they occur.
